Job summary

Churches of Christ in Queensland is seeking a dedicated Support Practitioner to join the Children, Youth & Families team in the Mackay area. You will conduct comprehensive assessments, develop individualized care plans, and provide crisis intervention and advocacy to support children, young people and families.

You will work with internal teams and external agencies, ensure accurate records, and promote culturally safe services.

Qualifications

• Degree in social work, psychology, or related field (or equivalent experience).
• Experience in child protection, family support, or community services.
• Knowledge of child protection legislation and out-of-home care frameworks.
• Strong assessment, communication, and interpersonal skills.

Responsibilities

• Conduct thorough assessments of children, young people, and families to identify needs and risks.
• Develop and implement tailored care plans in collaboration with clients and stakeholders.
• Provide timely crisis intervention to ensure safety and stability.
• Advocate for clients to access necessary resources and services.
• Facilitate evidence-based parenting programs and workshops.
• Coordinate care with internal teams and external agencies.
• Maintain accurate and confidential records in line with organisational standards.
• Ensure services are culturally safe and inclusive for diverse communities.
