Job summary

Wesley Mission Queensland Limited's STARH Program seeks a dedicated Case Manager to support individuals and families at risk of homelessness. You will deliver holistic, trauma-informed case management, advocacy and early intervention to help clients sustain tenancies and improve wellbeing.

You will collaborate with community partners, develop client-centred plans and maintain thorough records while navigating a dynamic, supportive team environment.

Qualifications

• Degree qualification in Human Services, Social Work, Community Services or related discipline.
• Experience in case management or community services.
• Experience using trauma-informed and person-centred practice.

Responsibilities

• Deliver holistic, trauma-informed case management, advocacy and support to individuals and families at risk of homelessness.
• Develop, implement and review client-centred support plans to achieve housing and wellbeing outcomes.
• Build relationships with clients in a respectful, culturally responsive manner.
• Coordinate services with community partners, stakeholders and support networks to meet client needs.
• Maintain accurate case notes, client records, reports and data in line with program and funding requirements.
• Manage a diverse caseload, prioritise tasks and respond to emerging client needs.
• Contribute to a collaborative team culture through case reviews, meetings and supervision.
