Wesley Mission Queensland Limited is seeking a Part-Time Case Manager to deliver outreach-based case management for children, parents and carers affected by substance misuse. You will work within a strengths-based, trauma-informed framework to build resilience and improve family wellbeing.

Role involves assessments, case planning, advocacy, and collaboration with Child Safety and health services. Tertiary qualification in Human Services or related field and valid drivers licence required.

• Relevant tertiary qualification in Human Services, Social Work, Psychology, Community Services, or related discipline.
• Current probity checks required by WMQ Limited policy.
• Current Driver’s Licence and willingness to travel for work.

• Conducting assessments and developing case plans to improve outcomes for children and families.
• Providing outreach, counselling, and therapeutic support to strengthen wellbeing and parenting capacity.
• Coordinating access to services, programs, support groups and community resources.
• Advocating for families and helping navigate complex service systems to achieve goals.
• Collaborating with Child Safety, schools, health services and other stakeholders for coordinated support.
• Maintaining accurate case notes, assessments, reports, client records and program data.
• Contributing to service development and a positive, collaborative team environment.

Your Role as Case Manager – Kids in Focus

The Kids in Focus Case Manager provides outreach-based support to children, parents, and family carers affected by substance misuse. Working from a strengths-based, trauma-informed and child-focused framework, you will support families to build resilience, strengthen parenting capacity, improve wellbeing, and create safe, nurturing environments where children can thrive.

A typical day may include:

• Delivering strengths-based, trauma-informed case management, support, and advocacy for children, parents, carers, and families affected by substance misuse.
• Conducting assessments and develop, implement, and review case plans focused on improving outcomes for children and families.
• Providing outreach, counselling, and therapeutic support to strengthen wellbeing, parenting capacity, and family relationships.
• Supporting families to access appropriate services, programs, support groups, and community resources.
• Advocating for families and assist them to navigate complex service systems and achieve their goals.
• Collaborating with Child Safety, schools, health services, and other stakeholders to coordinate effective support and referrals.
• Maintaining accurate case notes, assessments, reports, client records, and program data in line with organisational requirements.
• Contributing to service development, stakeholder engagement, and a positive, collaborative team environment.

This is a Part-Time (27.5 hours per week) Fixed-Term position until 30/06/2027.

What We’re Looking For

Demonstrated experience delivering holistic case management, assessment, advocacy, and referral services to individuals and families with complex needs. Experience supporting families affected by substance misuse, mental health challenges, or other social and family issues using a strengths-based approach. Strong communication, relationship-building, and stakeholder engagement skills, with the ability to work collaboratively across community and human services networks. Ability to work autonomously and within a team, supported by sound administrative, record-keeping, and computer skills, including Microsoft Office and client management systems.

Essential Requirements

• Relevant tertiary qualification in Human Services, Social Work, Psychology, Community Services, or a related discipline.
• Relevant probity checks required by legislation and WMQ Limited policy.
• Current Driver’s Licence and willingness to travel for work.
