Wesley Mission Queensland on the Gold Coast is seeking a Part-time Case Manager for the Youth & Health Education Service. You will provide client-centred case management, advocacy and support to help young people achieve their goals.

The role includes developing personalised support plans, delivering youth-friendly wellbeing guidance and referrals, facilitating educational programs, and coordinating access to community resources.

Formación

• Completed degree in relevant social welfare matters.
• Understanding of family relationships involving adolescents.
• Commitment to social justice, confidentiality and privacy.
• Experience establishing and maintaining agency networks within the human service environment.
• Current driver's licence and willingness to travel for work.

Responsabilidades

• Provide client-centred case management, advocacy and support to help young people achieve their goals and address their individual needs.
• Develop, implement and review personalised support plans in collaboration with clients.
• Deliver youth-friendly support, including emotional wellbeing guidance and referrals to appropriate services.
• Facilitate educational programs, workshops and health promotion activities that meet the needs of the target group.
• Build and maintain strong relationships with community services, stakeholders and partner organisations to support positive client outcomes.
• Coordinate access to support services and community resources, ensuring clients receive timely and appropriate assistance.
• Maintain accurate case notes, client records, reports and program statistics in line with organisational requirements.
• Respond effectively to crisis situations, conduct client assessments and work collaboratively within a multidisciplinary team to achieve program goals.
• Conduct community outreach across the Gold Coast area.
