Job summary

Centacare North Queensland seeks a dedicated outreach-case management professional in Mount Isa to support individuals and families who are homeless or at risk. You will assess needs, provide referrals, and advocate for secure housing options within a collaborative team.

The role involves building community partnerships, maintaining records, and participating in professional development opportunities. A driver’s license and screening clearances are required.

Qualifications

• Excellent written and verbal communication skills
• Strong analytical skills
• Current Driver's Licence
• Eligibility for Disability Worker Screening and Working with Children Blue Card

Responsibilities

• Provide case management support and develop early intervention strategies
• Conduct intake, triage and assessment to determine risk, urgency and eligibility
• Maintain participant records in electronic data systems per policies
• Develop partnerships through referrals and stakeholder engagement
• Attend meetings, supervision and professional development
• Undertake outreach and community engagement activities
• Contribute to program development and community education
• Comply with WHS, privacy and EEO legislation and quality systems
