Job summary

Warrigal seeks a Care Management and Clinical Leader to direct our Home Services, delivering safe, person-centred care with strong clinical governance and evidence-based practice.

You will align planing, oversight and workforce capability with client needs, while leading multidisciplinary teams and driving continuous improvement across our services to enhance wellbeing and independence at home.

Qualifications

• Tertiary qualification in health (Bachelor of Nursing, Allied Health or equivalent) with current APHRA registration.
• Demonstrated experience in clinical leadership or case management within aged care, home care or community services.
• Experience leading multidisciplinary teams including nursing, allied health professionals and care managers.
• Highly developed communication, stakeholder engagement and relationship management skills.
• Strong understanding of clinical governance, quality and risk management frameworks.
• Demonstrated ability to interpret client and operational data to improve service outcomes.

In this role, you will lead the care management and clinical functions across our Home Services, delivering safe, effective and person-centred care through strong clinical governance, professional practice and evidence-based approaches. Your leadership will ensure clients receive high-quality support that promotes wellbeing, independence and quality of life at home.

Partnering closely with operational leaders, you will align care planning, clinical oversight and workforce capability with client needs and service demand. You will contribute to the development of integrated care models, strengthen service performance and support the ongoing growth of Home Services.

Leading multidisciplinary teams, you will foster a culture of accountability, collaboration and continuous improvement. Through your expertise and influence, you will enhance outcomes for clients, families and communities while helping shape the future of home-based care

The requirements | What it takes to do this work
