Warrigal Home Services in Goulburn is seeking a skilled Care Coordinator to support the delivery of person-centred home care. You will coordinate and manage the care needs of community‑dwelling clients, ensuring services align with care plans, client goals and approved frameworks.
You will undertake assessments, develop and review care plans, balance a caseload and work with families, providers and internal teams to ensure safe, efficient services that promote independence and wellbeing.
You will play a pivotal role in supporting the delivery ofhigh-quality, person-centred care through Warrigal Home Services bycoordinating and managing the care needs of clients living in thecommunity. Working closely with the Care Services Manager,Operations Partner and broader Home Services team, you will ensureservices are aligned to individual care plans, client goals andapproved support frameworks.
This role requires someone who is highly organised, responsiveand confident managing competing priorities in a dynamicenvironment. You will be responsible for undertaking comprehensiveassessments, developing and reviewing care plans, and ensuringservices remain responsive to changing client needs. Your abilityto balance multiple client caseloads while maintaining a strongfocus on outcomes and wellbeing will be essential.
You will collaborate closely with clients, families, serviceproviders, and internal teams including Service Planning andRostering, to ensure services are delivered safely, efficiently andin line with care plans. Your abilityto problem-solve, communicateeffectively and respond proactively will be key to minimisingservice disruptions and supporting positive outcomes forclients.
Through proactive care management, regular monitoring andongoing engagement, you will support older people to live safelyand confidently in their own homes and communities. You will alsocontribute to continuous improvement by identifying opportunitiesto enhance care delivery, promote independence and ensure theeffective use of client funding to achieve optimal outcomes.
