Job summary

St Vincent's Care at Home in Australia is seeking an experienced Care Coordinator to join the national SVVH Programs team. You will manage referrals, onboard clients, coordinate services and monitor care plans, enabling seamless virtual and home-based care across Australia.

Work with clinicians, hospitals and service providers to improve patient outcomes at scale, with autonomy and national program objectives.

Qualifications

• Experience coordinating care within health services.
• Proven ability to manage referrals and care pathways.
• Excellent communication with clinicians and patients.

Responsibilities

• Coordinate patient journeys including referrals, eligibility screening, onboarding and service navigation.
• Provide direct client care coordination and support management of clinical escalations.
• Manage program enquiries and referrals, assess client eligibility and suitability.
• Monitor patient care plans and support service delivery through digital health platforms.
• Act as a subject matter expert and superuser for virtual care technologies and digital care systems.
• Collaborate with healthcare professionals, service providers and stakeholders to deliver integrated care.
• Coordinate subcontracted services to ensure continuity of care and positive outcomes.
• Maintain accurate clinical documentation, records and activity data per regs.
• Participate in quality improvement initiatives and program evaluation.
• Work with autonomy while supporting national program objectives.
