St Vincent’s Health Australia is recruiting for the State Manager - Care at Home (QLD/NSW), based in Brisbane or Sydney. You will lead the Care at Home portfolio, drive growth of virtual care and home-based services, and partner with hospitals, insurers and government partners to scale innovative models of care.

You will bring 7+ years of senior healthcare leadership, strong commercial acumen, and a track record of delivering transformational programs.

Qualifications

Senior healthcare leadership experience leading complex operations.
Proven track record in multidisciplinary teams and partnerships.
Experience with care-at-home, virtual care or Hospital-in-the-Home models is desirable.

Responsabilités

Lead the delivery, growth and optimisation of Care at Home services across Queensland.
Provide strategic and operational leadership across clinical and operational teams.
Drive growth initiatives and expand services and market presence.
Partner with hospitals, insurers and funding partners to strengthen referral pathways.
Collaborate with clinical, product and growth teams to scale home-based care models.
Oversee workforce planning and operational performance across service streams.
Monitor metrics to drive performance and continuous improvement.
Ensure clinical governance, risk management and accreditation standards.
