Job summary

Outcomes Connect Australia is seeking a Behaviour Support Practitioner to support people with disability, focusing on quality of life and rights. You will work with a team including clinical advisors to apply evidence-based practice and grow your own skills.

You live in or near Frankston and benefit from the School of Positive Behaviour Support partnership, with ongoing supervision and development opportunities.

Qualifications

• Current suitability (registration) with the NDIS Commission as a Behaviour Support Practitioner is preferred but not essential.
• Considerable practical experience supporting people with complex behaviours is essential.
• Demonstrated ability to work positively as part of a diverse team.
• Outstanding communication, influencing and interpersonal skills.
• Flawless report writing skills.

Responsibilities

• Support clients (NDIS participants) and families to identify needs via assessment and planning.
• Identify meaningful goals and strategies using PBS framework to reduce restrictive practices.
• Develop, deliver and evaluate evidence-based support plans in a complex space.
• Ensure plans reflect clients' goals, passions and strengths.
• Build knowledge and capacity of family, carers and supporters.
