Job summary

Outcomes Connect Australia is seeking a Behaviour Support Practitioner to improve quality of life for people living with disability. You will work in a team with practitioners, a team leader and clinical advisors to deliver evidence-based support and grow your knowledge and skills.

You live in or near Warrnambool and may participate in the PBS program with MedHealth Academy and the University of Melbourne. Flexible scheduling and remote/community-based work are offered.

Qualifications

• Current suitability/registration with the NDIS Commission as a Behaviour Support Practitioner is preferred but not essential.
• Considerable practical experience in supporting people with complex and challenging behaviours is essential.
• Demonstrated ability to work positively as a member of a diverse team.
• Outstanding communication, influencing and interpersonal skills; flawless report writing.
• Current Drivers Licence and own reliable vehicle.

Responsibilities

• Support clients (NDIS participants) and their families to identify needs through assessment and person-centred planning.
• Identify meaningful goals and practical strategies using the Positive Behaviour Support framework.
• Develop, deliver and evaluate evidence-based support plans within a complex space.
• Ensure individual plans reflect clients' goals, passions and strengths.
