Gateway Health is seeking a Non-Clinical Aged Care Needs Assessor - Home Support based in Wodonga for a 12-month, part-time temporary role. You will perform face-to-face assessments of older people to access Commonwealth subsidised aged care services, determine needs, and coordinate supportive options to promote independence and wellbeing.

The role is classified under the Social and Community Service Employees Multi Enterprise Agreement 2022, level 4, with hourly rates of $46.70–$50.25 plus

Qualifications

• Experience in assessment, referral, case management or carer support.
• Knowledge of aged care service system, wellness and reablement principles.
• Advanced clinical expertise in continence and/or urology nursing.
• Strong communication and report writing skills.

Responsabilités

• Provide high-quality assessments to older people seeking access to subsidised aged care services.
• Identify appropriate support options and facilitate access to services.
• Conduct mainly face-to-face assessments in the client’s home.
• Prepare reports and correspondence tailored to purpose and audience.
