Job summary

Melbourne Health’s Royal Melbourne Hospital Aged Care Assessment team seeks a Home Support Assessor to help older people access the support and services they need to live well at home and in their community. You will complete non-clinical assessments in homes, by phone or via telehealth, develop individualised support plans, and connect clients with appropriate aged care services under supervision.

Qualifications

• Holds a Cert IV in aged care/community services or equivalent.
• Experience in assessment-based role within aged or community care.

Responsibilities

• Provide respectful support to older people in their homes, by phone or via telehealth.
• Complete the Integrated Assessment Tool (IAT) with consumers to capture needs.
• Develop individualised support plans reflecting goals, strengths and preferences, and identify services via My Aged Care.
• Seek guidance and elevate concerns to senior clinical staff as required.
• Work under supervision of a Grade 3 Allied Health Professional or Grade 4 Nurse.
