Job summary

Community Options Australia is seeking compassionate aged care assessors to conduct face-to-face assessments with older Australians in their homes across ACT. You will determine eligibility for supports under the My Aged Care program and develop person-centred care plans.

We offer flexible part-time opportunities, training, and a supportive team environment while you help clients stay independent in their homes and communities.

Qualifications

• Degree in Allied Health and/or Social Work.
• Registration with AHPRA or membership of a relevant professional association.
• High level of digital and computer literacy.
• Current Influenza vaccination and National Police/Working with Children checks as applicable.

Responsibilities

• Conduct client assessments in clients' homes using a wellness and reablement approach.
• Develop person-centred support plans.
• Review and monitor ongoing client needs.
• Liaise with referral agencies and community service providers.
• Maintain accurate records using the My Aged Care portal.
