Zusammenfassung

IPC Health is seeking a Part Time Aged Care Assessor to deliver Home Support Aged Care assessments across Hobsons Bay, Brimbank, Melton and Wyndham. You will conduct holistic assessments for clients 65+ (50+ for Aboriginal or Torres Strait Islander people) using the Integrated Assessment Tool via My Aged Care.

You will develop collaborative Support Plans to promote independence, empower clients, and ensure data accuracy, quality care and regulatory compliance while contributing to service

Qualifikationen

• Current or recent experience as an Aged Care Assessor is highly desirable.
• Ability to conduct comprehensive, holistic client assessments and develop individualised support plans.
• Excellent time management and organisational skills.
• Strong communication to engage with clients and work in a multidisciplinary environment.

Aufgaben

• Manage an assessment caseload and conduct Home Support Aged Care assessments.
• Undertake home assessments in client homes or chosen locations using the Integrated Assessment Tool via My Aged Care.
• Develop collaborative Support Plans to enable independence and community living.
• Empower clients by tailoring services to individual needs and goals.
• Maintain data accuracy and comply with quality and regulatory standards.
