Job summary

Gateway Health in Wangaratta, Victoria, is hiring two positions (full-time or part-time) to deliver the Aged Care Assessment Program (ACAP). The focus is to assess eligibility for government-subsidised aged care and support older people to access appropriate services.

The role is classified under the SACS award at Level 5 ($51-$53.31/hour plus superannuation). We offer flexible hours, paid parental leave, NFP Salary Packaging up to $15,900 per year, a laptop and mobile phone with remote access,

Qualifications

• Tertiary qualification in a health-related discipline.
• Current unrestricted AHPRA registration or eligibility.
• Minimum of one year demonstrated experience in aged care or related settings, including dementia care.
• Current MAC Learning training modules or willingness to complete.

Responsibilities

• Conduct ACAP assessments to determine eligibility for government-subsidised aged care.
• Deliver services and support to older persons in aged care settings.
