IPC Health Ltd. is recruiting for a Part Time Aged Care Assessor to support the SAS program across Hobsons Bay, Brimbank, Melton and Wyndham. The role focuses on holistic assessments, collaborative care planning and helping older clients live independently at home.

You will manage a caseload, use My Aged Care tools and maintain accurate client data, with travel across multiple LGAs and an option to work from home where feasible. A fixed term until 30 June 2027 applies.

Qualifications

• Current or recent experience as an Aged Care Assessor is highly desirable.
• Ability to conduct holistic client assessments and develop individualised support plans.
• Excellent time management and strong communication in a multidisciplinary setting.
• Maintain accurate client data and use My Aged Care portal effectively.
• Identify and manage risks while meeting program KPIs.

Responsabilités

• Manage an assessment caseload and conduct Home Support Aged Care assessments.
• Undertake home assessments in client homes or other settings using the Integrated Assessment Tool.
• Develop collaborative Support Plans to support independence at home.
• Empower clients by involving them in planning and tailoring services.
• Ensure compliance with standards and regulatory requirements.
