Job summary

Community Options Australia is seeking compassionate Aged Care Assessors to perform in-home assessments across the Southern Highlands, determining eligibility for supports under the My Aged Care program and helping clients remain independent at home and in their communities.

You will work autonomously within your region, reporting to the Assessment Team Lead, and collaborate with referral agencies to coordinate services.

Qualifications

• Degree in Allied Health and/or Social Work or equivalent.
• Registration with AHPRA or membership of a relevant professional association (eg AASW).
• High level of digital and computer literacy.
• National Criminal History Check & Working with Children Check (NSW or equivalent).
• Current Influenza vaccination (or willingness to update).
• Strong verbal and written English communication skills are essential for this role, alongside the ability to engage respectfully across cultures.
• Current Australian Driver's License and own reliable, insured vehicle (Vehicle Allowance provided under SCHADS Award).
• Understanding that daily driving within their area will be a part of the role.

Responsibilities

• Conduct client assessments in their homes using a wellness and reablement approach.
• Develop person-centred support plans.
• Review and monitor ongoing client needs.
• Liaise with referral agencies and community service providers.
• Maintain accurate records using the My Aged Care portal.
