Job summary

Western District Health Service in Hamilton, Victoria, is seeking a compassionate Aged Care Services Intake Officer to coordinate referrals, assess needs and develop care plans for the Support at Home, CHSP and HACC-PYP programs. The role involves client engagement, service coordination and ongoing care plan reviews.

You will work with a multidisciplinary team to connect older people with appropriate supports, maintaining records in My Aged Care and other systems.

Qualifications

• Previous experience in aged care, community services, intake, assessment, care coordination or related field.
• Strong interpersonal and relationship-building skills.
• Excellent written and verbal communication abilities.
• Demonstrated ability to conduct assessments and develop person-centred care plans.
• High level organisational skills and attention to detail.
• Familiarity with client management systems and electronic records.
• Ability to discuss service costs and client contributions professionally and sensitively.
• Current Driver's Licence.
• Current Police Check (or willingness to obtain).
• Rights to work in Australia.

Responsibilities

• Conduct comprehensive client assessments to identify goals, needs and risks and develop person-centred care plans.
• Manage referrals across programs ensuring timely access to supports.
• Build positive relationships with clients, families and carers throughout intake and assessment.
• Monitor client needs through regular care plan reviews and adjust services as required.
• Maintain accurate client records and care plans using systems such as My Aged Care, eSAH and PRODA.
• Collaborate with internal teams and external providers to coordinate service delivery and drive continuous improvement.
