Job summary

Just for u Care, a Melbourne-based in-home care provider, is seeking a warm, organised Intake Officer to be the first point of contact for older people and families seeking support at home. The role blends office work with visits to clients’ homes for assessments and sign-ups, ensuring smooth handovers to Care Consultants from day one.

Qualifications

• Experience in intake, client services or care coordination, ideally in aged care or community services.
• Calm, friendly manner on the phone and ability to handle sensitive conversations.

Responsibilities

• Respond promptly to enquiries by phone and email from older people, families, hospitals, assessors and referrers.
• Explain services and how Support at Home funding works in plain language.
• Gather client information, confirm classification and funding, and complete intake paperwork.
• Coordinate service agreements and onboarding documentation.
• Visit new clients in their homes to complete assessments and sign-ups.
• Hand new clients over to the appropriate Care Consultant with all necessary materials.
• Maintain accurate records in the client management system and follow up on inquiries.
• Build relationships with referrers, assessors and community partners.
