Job summary

Western District Health Service in Hamilton, VIC, is seeking an experienced Team Leader Care Coordination to lead a small team providing in-home aged care services while maintaining a hands-on caseload.

Reporting to the Manager In Home & Community Operations, you will mentor Care Partners and Intake Officers, drive quality, compliance and service delivery, and help implement aged care reforms and continuous improvement.

Qualifications

• Degree, Diploma or Certificate in a Community, Nursing, Health or Management discipline, or equivalent experience
• Previous experience as a Case Manager within the Support at Home environment desirable
• Comprehensive knowledge of Support at Home Program legislation and funding
• Demonstrated leadership, mentoring and staff development capability
• Strong communication, relationship-building and stakeholder engagement skills
• Experience managing quality and compliance requirements
• Sound organisational and operational management skills
• Strong problem-solving and continuous improvement capability

Responsibilities

• Provide leadership, mentoring and support to Care Partners and Intake Officers while fostering a positive, accountable and high-performing team culture.
• Support onboarding, workforce development, coaching and supervision activities.
• Maintain a small participant caseload to support service delivery and contemporary practice, while overseeing participant allocation processes and ensuring services commence in a timely and effective manner.
• Monitor service quality and compliance through audits, documentation reviews and quality assurance activities, ensuring services meet legislative and organisational requirements.
• Support daily operations through workflow management, rostering, problem-solving and process improvement activities that enhance team performance and participant outcomes.
• Lead incident, complaint and risk management processes, ensuring issues are investigated, documented, resolved and escalated appropriately.
• Support implementation of aged care reforms, quality initiatives and service improvements that strengthen compliance, efficiency and participant outcomes.
