Job Overview

GluCare Integrated Diabetes Center is seeking a dedicated DHA Registered Nurse to join our clinical team in delivering exceptional patient care. The ideal candidate is compassionate adaptable and committed to providing highquality nursing services in a modern fastpaced environment. Experience in chronic disease management patient education and digital tools is a plus.

Key Responsibilities
Patient Care Support
  • Provide direct patient care including vital sign checks blood glucose monitoring and patient observations.
  • Educate patients on diabetes management including lifestyle tips and monitoring instructions ensuring compliance with care plans.
Clinical Procedures Assistance
  • Prepare examination rooms and medical equipment ensuring cleanliness and adherence to infection control standards.
  • Assist in the preparation and handling of equipment for procedures wound care and specimen collection.
Patient Education and Communication
  • Communicate effectively with patients providing empathy and clarity to address their questions and concerns.
  • Reinforce health education provided by physicians encouraging selfcare practices and adherence to treatment plans.
Documentation and Reporting
  • Accurately record and update patient information treatment plans and observations in electronic medical records (EMR).
  • Report any changes in patient condition to the supervising nurse or physician promptly.
Operational and Safety Compliance
  • Adhere to all DHA regulations nursing guidelines and GluCare protocols to ensure the safety and wellbeing of patients and staff.
  • Participate in ongoing training and development programs to stay current with best practices in diabetes care and nursing support.
Requirements
