Qualifications

• Excellent communication, negotiation, and interpersonal abilities for remote collaboration.

Responsibilities

• Assess patient needs and develop comprehensive, individualized care plans in collaboration with healthcare teams.
• Coordinate patient care services, ensuring continuity across providers and facilities.
• Advocate for patients' rights and preferences throughout their healthcare journey.
• Liaise with insurance providers to obtain authorizations for services and manage coverage issues.
