The ideal candidate has 5+ years of clinical nursing experience, with 2+ years in case management, strong clinical judgment, and proven experience with EHR and telehealth in a remote work setup.

Formación

• Current and unrestricted Registered Nurse license.
• At least 5 years clinical nursing experience; 2+ years in case management.
• Strong knowledge of healthcare regulations and insurance processes.
• Excellent assessment, planning and problem-solving skills.
• Proficient with EHR systems and telehealth platforms.
• Strong remote collaboration and communication skills.

Responsabilidades

• Assess patient needs and develop care plans with healthcare teams.
• Coordinate patient care across providers and facilities.
• Advocate for patients' rights and preferences throughout care.
• Liaise with insurers for authorizations and coverage issues.
• Monitor progress and adjust care plans as needed.
• Educate patients and families about options, resources, and discharge planning.
