Job summary

Edgexec is seeking a Senior Registered Nurse specializing in Case Management for a fully remote role. You will coordinate patient care, work with interprofessional teams, and navigate insurance processes to optimize outcomes.

The position requires 5+ years nursing experience with 2+ years in case management, strong clinical judgment, and proficiency with EHR and telehealth platforms. Remote work offers flexible balance and ongoing professional growth.

Qualifications

Current and unrestricted Registered Nurse license.
Minimum of 5 years of clinical nursing experience, with at least 2 years in case management or care coordination.
Strong understanding of healthcare regulations, insurance processes, and community resources.

Responsibilities

Assess patient needs and develop comprehensive, individualized care plans in collaboration with healthcare teams.
Coordinate patient care services, ensuring continuity and efficiency across different providers and facilities.
Advocate for patients' needs, rights, and preferences throughout their healthcare journey.
Liaise with insurance providers to obtain authorizations for services and manage coverage issues.
Monitor patient progress, evaluate the effectiveness of care plans, and make necessary adjustments.
Educate patients and families about treatment options, available resources, and discharge planning.
