Edengph is seeking a highly experienced Senior Registered Nurse specializing in Case Management for a fully remote role. You will coordinate patient care, navigate insurance processes, and advocate for patients across settings.

The ideal candidate has 5+ years of clinical nursing with at least 2 years in case management, strong clinical judgement, excellent communication, and proficiency with EHR and telehealth platforms.

Qualifications

• Current and unrestricted Registered Nurse license.
• Minimum 5 years clinical nursing experience with at least 2 in case management or care coordination.
• Strong understanding of healthcare regulations and insurance processes.
• Excellent assessment, planning and communication skills.

Responsibilities

• Assess patient needs and develop individualized care plans with healthcare teams.
• Coordinate patient care across providers and facilities.
• Advocate for patient rights and preferences throughout care.
• Liaise with insurers to obtain authorizations and manage coverage.
• Monitor progress and adjust care plans as needed.
• Educate patients and families about options and discharge planning.
