Clinix Health Group is seeking a highly experienced Senior Registered Nurse with a specialization in Case Management for a fully remote role. You will coordinate patient care, manage resources, advocate for patients, and liaise with insurers to obtain authorizations across settings. This position requires 5+ years in clinical nursing with at least 2 years in case management, and proficiency with EHR and telehealth.

Formación

• Current and unrestricted nursing license required.
• Minimum of 5 years of clinical nursing experience with at least 2 years in case management or care coordination.
• Proficiency with EHR systems and telehealth platforms.

Responsabilidades

• Assess patient needs and develop care plans with healthcare teams.
• Coordinate patient care across providers and facilities.
• Advocate for patients' needs and preferences.
• Liaise with insurance providers for authorizations.
• Monitor progress and adjust care plans as needed.
• Educate patients and families about treatment options and discharge planning.
