Qualifications

• Excellent assessment, planning, and problem-solving abilities; proficient with EHR and telehealth platforms.

Responsibilities

• Assess patient needs and develop comprehensive, individualized care plans with care teams.
• Coordinate care across providers and facilities to ensure continuity.
• Advocate for patients' rights and preferences throughout their healthcare journey.
• Liaise with insurers to obtain authorizations and manage coverage issues.
• Monitor progress, evaluate care plans, and adjust as needed.
• Educate patients and families about treatment options and discharge planning.
