Discovery Limited is seeking a Senior Registered Nurse specializing in Case Management for a fully remote role. The position focuses on coordinating patient care across settings, managing resources, and advocating for patient needs with families, providers, and insurers.

The ideal candidate has 5+ years of clinical nursing experience with at least 2 years in case management, strong organizational and clinical judgment, and proficiency with EHR and telehealth platforms to support remote care

Qualifications

• Current and unrestricted Registered Nurse license.
• 5+ years of experience in clinical nursing, with at least 2 years in case management or care coordination.
• Strong understanding of healthcare regulations, insurance processes, and community resources.
• Excellent assessment, planning, and problem-solving skills.
• Proficient in using electronic health record (EHR) systems and telehealth platforms.
• Exceptional communication, negotiation, and interpersonal abilities for remote collaboration.

Responsibilities

• Assess patient needs and develop comprehensive, individualized care plans in collaboration with healthcare teams.
• Coordinate patient care services, ensuring continuity and efficiency across different providers and facilities.
• Advocate for patients' needs, rights, and preferences throughout their healthcare journey.
• Liaise with insurance providers to obtain authorizations for services and manage coverage issues.
• Monitor patient progress, evaluate the effectiveness of care plans, and make necessary adjustments.
• Educate patients and families about treatment options, available resources, and discharge planning.
