Job summary

Netcare is seeking a Senior Registered Nurse specialized in Case Management to join our remote healthcare team. This role focuses on coordinating patient care, optimizing resources, and ensuring positive outcomes across settings.

You will collaborate with patients, families, providers, and insurers to navigate the healthcare system and advocate for patient needs. The ideal candidate has at least 5 years of clinical nursing experience, with 2 years in case management, and is proficient with EHR

Qualifications

Current and unrestricted Registered Nurse license.
Minimum of 5 years clinical nursing experience, with at least 2 years in case management or care coordination.
Proficient in using electronic health record (EHR) systems and telehealth platforms.

Responsibilities

Assess patient needs and develop comprehensive, individualized care plans in collaboration with healthcare teams.
Coordinate patient care services, ensuring continuity and efficiency across different providers and facilities.
Advocate for patients’ needs, rights, and preferences throughout their healthcare journey.
Liaise with insurance providers to obtain authorizations for services and manage coverage issues.
Monitor patient progress, evaluate the effectiveness of care plans, and make necessary adjustments.
Educate patients and families about treatment options, available resources, and discharge planning.
