Placements24 is seeking a Senior Case Manager for our client in Cape Town. The role focuses on providing intensive, person-centered support to vulnerable adults, coordinating housing, healthcare, employment and legal aid, and advocating for client independence and stability.

You will lead assessments, plan coordinated services, monitor progress, respond to crises with empathy, and mentor junior case managers.

Qualifikationen

• Master's degree in Social Work, Psychology, or a related field.
• Minimum 7 years of direct case management experience.
• Experience with vulnerable adult populations preferred.
• Strong assessment, crisis intervention, and resource coordination skills.
• Knowledge of Cape Town context and relevant policies.

Aufgaben

• Manage a caseload of vulnerable adults and provide intensive case management.
• Conduct thorough assessments of client needs and develop individualized plans.
• Coordinate access to housing, healthcare, employment, and legal aid.
• Provide emotional support, counseling, and advocacy for clients.
• Liaise with external agencies, healthcare providers, and stakeholders in Cape Town.
• Monitor client progress and adjust plans to ensure continuity of care.
• Maintain detailed client records per standards and regulations.
• Respond to and manage crisis interventions with professionalism and sensitivity.
• Mentor and support junior case managers, sharing expertise.
• Participate in team meetings, case conferences, and service development initiatives.
