Placements24 in East London is seeking a dedicated Case Manager to support vulnerable populations, including the homeless, at-risk youth, and families in crisis. You will assess client needs, develop support plans, and coordinate access to services within the Eastern Cape.

The role requires strong advocacy and compassionate approach to help marginalized communities, with collaboration across service providers and ongoing professional development.

Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology, or related field.
• Minimum of 3 years of experience in case management or direct service provision.
• Knowledge of social service systems and Eastern Cape legislation.
• Valid driver's license and access to a reliable vehicle.
• Strong interpersonal and communication skills.
• Ability to write clear, concise reports and maintain confidential records.

Responsibilities

• Conduct psychosocial assessments identifying client needs, strengths and barriers.
• Develop and implement individualized case management plans with clients.
• Coordinate access to housing, healthcare, employment and social services.
• Provide direct support, advocacy and crisis intervention.
• Maintain accurate, confidential client records and progress notes.
• Collaborate with service providers and stakeholders.
• Monitor progress and adjust service plans as needed.
• Ensure compliance with policies, procedures and legal requirements.
• Connect clients with life skills, education and vocational resources.
• Participate in case conferences, meetings and professional development.
