Ideal candidates will have 5+ years in case management, a relevant advanced degree, and experience with diverse communities, plus proficiency in electronic case management systems.

Qualifications

• Master's degree preferred.
• Minimum of 5 years of progressive experience in case management or social services.
• Experience working with diverse and vulnerable populations.
• In-depth knowledge of the social service infrastructure and resources in Cape Town.

Responsabilités

• Conduct in-depth assessments of client needs, identifying strengths, challenges and goals.
• Develop, implement and regularly review individualized case plans for clients.
• Coordinate with healthcare, housing, education and vocational programs.
• Provide direct support, counseling and crisis intervention.
• Monitor progress and adjust care plans as needed.
• Maintain confidential client records and documentation.
• Advocate for clients with service providers to meet rights and needs.
• Collaborate with internal teams and external agencies for referrals.
• Supervise and mentor junior case managers.
• Contribute to program development and quality improvement initiatives.
