Placements24 is seeking a highly organized Rehabilitation Coordinator to support clients in Mossel Bay and surrounding areas from a remote-first team. You will develop rehabilitation plans, coordinate with healthcare providers, and conduct regular virtual check-ins to monitor progress.

The role emphasizes remote collaboration, excellent organization, and strong client communication to ensure timely services and successful reintegration within the Western Cape community.

Qualifications

• Bachelor's degree in a relevant field.
• BSc/BA in Social Work, OT, Psychology or Health Administration preferred.
• Experience in case management or program coordination within rehab or healthcare.
• Strong rehab principles & knowledge of community resources.
• Excellent organizational and time-management skills with multi-case handling.
• Proficient with remote communication tools and EHR systems.
• Strong interpersonal and virtual communication with clients.

Responsibilities

• Develop and monitor individualized rehabilitation plans.
• Coordinate services with healthcare providers and community agencies.
• Conduct regular virtual check-ins and progress assessments.
• Maintain detailed client records in a secure cloud system.
• Serve as liaison between clients, families, and support networks.
• Identify barriers to rehabilitation and develop mitigation strategies.
• Ensure compliance with regulations and ethical guidelines.
