Placements24 is seeking a skilled Remote Social Care Specialist to provide virtual support to individuals and families from home. You will assess needs, offer counseling, and connect clients with resources across regions, with a focus on the North West province.

The role requires a relevant degree (Social Work or Psychology), at least 3 years' remote care experience, fluency in digital platforms like Zoom or Teams, and strong written and verbal communication.

Qualifications

• A relevant degree in Social Work, Psychology, or a related field.
• Minimum of 3 years’ experience in a social care or counseling role.
• Proficiency with virtual communication platforms (Zoom, Teams, etc.) and case management software.
• Strong understanding of social issues and resources across communities, including the North West region.
• Excellent written and verbal communication skills, with empathy and professionalism.

Responsibilities

• Conduct comprehensive client needs assessments via video conferencing and other digital tools.
• Provide virtual counseling, emotional support, and guidance to clients facing diverse challenges.
• Develop and manage individualized support plans, coordinating with local resources when necessary.
• Assist clients in navigating social welfare systems, healthcare services, and community programs remotely.
• Maintain accurate, secure, and confidential digital client records and progress notes.
• Facilitate online support groups and educational workshops for clients.
• Stay abreast of digital best practices in social care delivery and client support.
