Job summary

Placements24 is seeking a compassionate Remote Social Support Specialist to deliver essential services to clients across diverse locations. This fully remote role enables you to provide case management, resource navigation, and direct virtual support from your home office.

You will assess needs, guide clients to healthcare, housing, finances, and employment opportunities, and maintain meticulous electronic case files while collaborating with a remote team.

Qualifications

• Relevant degree in Social Work, Psychology, Sociology or closely related field.
• Proven experience in social work, counseling, or case management, preferably with remote clients.
• Strong digital literacy, including proficiency with video conferencing tools and case management software.
• Excellent active listening, empathetic communication, and problem-solving skills.
• Ability to work independently, manage time efficiently, and maintain strict confidentiality.

Responsibilities

• Provide direct virtual support and case management to clients facing social and economic challenges.
• Conduct needs assessments via video conferencing and phone, developing personalized support plans.
• Assist clients in accessing essential services, including healthcare, housing, financial assistance, and employment opportunities.
• Maintain accurate, up-to-date electronic case files and document all client interactions and progress meticulously.
• Collaborate effectively with remote team members and external service providers through digital communication channels.
• Stay informed about available community resources and advocate for clients' needs within the support network.
