Placements24 is seeking a compassionate Social Support Facilitator to join their fully remote Community & Social Care team serving Bloemfontein. You will provide virtual guidance, support, and resource navigation to individuals and families facing social or personal challenges.

Qualifications

• Degree or certification in Social Work, Psychology, Counseling, or a related field.
• Minimum of 2 years of experience in a direct support or counseling role.
• Proven ability to communicate effectively and empathetically via phone, video conferencing, and email.
• Experience with case management software and virtual collaboration tools is essential.
• Strong understanding of community support services and referral processes.
• Excellent organizational skills and the ability to manage workload independently in a remote setting.

Responsibilities

• Conduct virtual client assessments to identify social, emotional, and practical needs.
