Job summary

Placements24 is seeking a Remote Rehabilitation Support Coordinator to provide comprehensive virtual support to clients navigating rehabilitation pathways from a home-based setup. Qualifications

• Bachelor's degree in Social Work, Occupational Therapy, Psychology, Disability Studies, or related field.
• Minimum of 3 years of experience in rehabilitation, disability services, or case management.
• Strong understanding of various disabilities and rehabilitation processes.
• Excellent virtual communication, interpersonal, and active listening skills.
• Proficiency in using video conferencing, case management software, and other digital tools for remote client interaction.
• High level of organizational skills and ability to manage multiple client cases concurrently.

Responsibilities

• Conduct virtual needs assessments and identify challenges across physical, cognitive, and psychosocial domains.
• Develop and implement personalized rehabilitation plans with clients and healthcare professionals.
• Coordinate access to services (therapy, counseling, assistive tech, vocational training) via virtual means.
• Provide ongoing virtual support, guidance, and motivation to clients.
• Liaise with healthcare providers, community organizations and government agencies to ensure seamless service delivery.
• Maintain confidential electronic client records and document progress and outcomes.
