Qualifications

• Bachelor's degree in Health Sciences, Social Work, or a related field.
• At least 3 years of experience in case management, rehabilitation services, or healthcare coordination.
• Strong understanding of rehabilitation principles and the healthcare system.
• Excellent organizational and interpersonal skills.
• Ability to work effectively in a remote capacity, supporting clients in and around Pretoria.

Responsibilities

• Coordinate and manage rehabilitation plans for clients, ensuring adherence to treatment protocols.
• Liaise with doctors, therapists, insurance providers, and family members to facilitate smooth service delivery.
• Assist clients in accessing rehabilitation services, equipment, and support networks.
• Monitor client progress and adjust care plans as necessary in consultation with the healthcare team.
