Job summary

Responsibilities

Conduct virtual assessments to understand clients' wellness needs and challenges.
Develop and implement personalized remote wellness and support plans.
Coordinate access to mental health services, counseling, social programs, and other wellbeing resources.
Provide ongoing virtual support, guidance, and encouragement to clients.
Utilize video conferencing, phone, and secure messaging for client engagement.
Maintain detailed, confidential electronic client records.
