Job summary

Placements24 is seeking a Remote Community Health Navigator in South Africa to connect underserved populations with vital health services from a home office. You will guide clients, advocate for equitable access, and support navigation through health systems using digital tools and remote communication.

Ideal candidates bring a Bachelor's in a health field, ≥3 years in health navigation or advocacy, and strong remote work skills.

Qualifications

• Bachelor's degree in Public Health, Social Work, Nursing, or related field.
• Minimum of three years of experience in community health, patient advocacy, or health navigation.
• Proficiency with telehealth platforms and electronic health record systems.
• Strong written and verbal communication; ability to work remotely.

Responsibilities

• Conduct virtual needs assessments to understand clients' health service requirements and barriers.
• Provide telephonic and online guidance on navigating healthcare systems, insurance, and available programs.
• Assist clients in scheduling appointments with healthcare providers and accessing necessary medical services.
• Educate community members on preventive health practices and available wellness resources through digital channels.
• Maintain accurate electronic health records and case management documentation.
• Collaborate remotely with healthcare providers, community organizations, and internal teams to ensure comprehensive client support.
