Job summary

The Gauteng Department Of Health is seeking a Senior Registered Nurse specializing in Case Management for a fully remote role. You will coordinate patient care, manage resources, and advocate for patient needs across settings, partnering with families, providers, and insurers to facilitate seamless transitions of care.

The ideal candidate has at least 5 years of clinical nursing experience, including 2 years in case management, strong clinical judgment, and proficiency with EHR and telehealth

Qualifications

• Minimum 5 years of clinical nursing experience, with at least 2 years in case management or care coordination.
• Proven ability to coordinate patient care across multiple settings and providers.
• Strong communication and negotiation skills for remote collaboration.

Responsibilities

• Assess patient needs and develop comprehensive, individualized care plans with healthcare teams.
• Coordinate patient care services to ensure continuity across providers and facilities.
• Advocate for patients’ needs, rights, and preferences throughout the care journey.
• Liaise with insurance providers to obtain authorizations and manage coverage issues.
• Monitor progress, evaluate outcomes, and adjust care plans as needed.
• Educate patients and families about treatment options, resources, and discharge planning.
