Placements24 is seeking a proactive Remote Case Manager to join our social services team and support clients from home in South Africa. You will manage client caseloads, perform needs assessments, and guide individuals and families toward relevant services while maintaining strict confidentiality.

The role emphasizes independent work, strong organization, and empathetic communication within a supportive virtual team. A bachelor’s degree in a related field and 2+ years’ experience are required.

Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology, or a related field.
• Minimum of 2 years' experience in case management or a similar social services role.
• Proficiency with remote communication tools (e.g., Zoom, Teams) and case management software.
• Demonstrated ability to work independently, manage time effectively, and maintain client confidentiality.
• Strong organizational and documentation skills, with meticulous attention to detail.
• Empathetic communication style and a commitment to client advocacy and empowerment.

Responsibilities

• Manage a diverse caseload of clients, providing ongoing support and case management services remotely.
• Conduct needs assessments via virtual platforms and phone consultations to identify client requirements.
• Develop, implement, and monitor personalized support plans, tracking client progress towards established goals.
• Connect clients with appropriate community resources and services, facilitating referrals and follow-ups.
• Maintain accurate and detailed electronic case files, ensuring compliance with organizational policies and privacy regulations.
• Collaborate with internal teams and external agencies to provide comprehensive client support.
