Placements24 is seeking a highly organized Remote Case Manager to support clients across South Africa from home. You will assess needs, develop care plans, and advocate within health and social services systems.

This role requires proven case management experience, expertise in navigating healthcare resources, strong assessment and problem-solving skills, and the ability to work independently using case management software.

Qualifications

• Proven experience as a Case Manager or in a similar role involving client support and coordination.
• Strong understanding of social services, healthcare systems, and community resources.
• Excellent assessment, problem-solving, and decision-making skills.
• Proficiency in using case management software and virtual communication tools.
• Exceptional organizational and time management abilities, with a capacity for independent work.
• Relevant professional qualifications or certifications in social work, counseling, or a related field are required.

Responsibilities

• Conduct comprehensive assessments of client needs, including psychosocial, medical, and financial aspects.
• Develop, implement, and monitor individualized case plans in collaboration with clients and relevant stakeholders.
• Coordinate access to community resources, healthcare services, and social support networks.
• Act as a primary point of contact and advocate for clients, ensuring their voices are heard and needs are addressed.
• Maintain accurate, detailed, and confidential client records in compliance with organizational policies and regulations.
• Regularly review case progress, adjusting plans as necessary to achieve client goals and promote independence.
