Placements24 is seeking a dedicated Case Manager to support vulnerable families in Soweto, Gauteng. You will assess needs, develop individualized plans, and connect families with housing, healthcare, education and employment resources.

This full-time role requires empathy, strong organizational skills and knowledge of local welfare systems. You will advocate for families, maintain confidential records, collaborate with service providers, and travel within Soweto to conduct home visits.

Qualifications

• A relevant qualification in Social Work, Psychology, Community Development, or a related field.
• Proven experience in case management, social services, or family support.
• Knowledge of social welfare systems and resources in Gauteng.
• Strong assessment, planning, and intervention skills.
• Excellent communication, interpersonal, and active listening abilities.
• Ability to travel within Soweto and conduct home visits.
• Commitment to empowering families and well-being.

Responsabilités

• Assess family needs and risks to inform care plans.
• Develop, implement, and monitor individualized case plans.
• Provide direct support, counseling and guidance to families.
• Refer families to housing, healthcare, education, and employment services.
• Advocate for families within systems.
• Maintain confidential case records.
• Collaborate with service providers and government agencies.
• Respond to crises and provide immediate intervention.
