Job summary

Placements24 is seeking a motivated Community Support Facilitator for the Mahikeng, North West area. The role combines field community engagement with remote coordination to connect residents with social services, education, health, and employment resources.

You will map community needs, develop partnerships with local organizations, and guide clients through available programs. The position requires strong communication, cultural sensitivity, and a willingness to travel within the Mahikeng

Qualifications

• A diploma or relevant certification in Community Development, Social Sciences, or a related field.
• Minimum of 2–3 years of experience in community work, social support, or outreach.
• Demonstrated ability to engage effectively with diverse community members.
• Strong knowledge of social services, government programs, and community resources in North West.
• Excellent communication, listening, and problem-solving skills.
• Ability to work both independently and collaboratively within a team.
• Valid driver's license and willingness to travel within the Mahikeng area.

Responsibilities

• Identify community needs and map available resources and support services.
• Facilitate connections between individuals/families and relevant service providers (e.g., health, education, employment).
• Provide guidance and information on accessing social welfare programs and benefits.
• Organize and conduct community outreach activities and awareness campaigns.
• Develop and maintain positive relationships with community leaders and local organizations.
• Assist clients in navigating bureaucratic processes to access services.
• Monitor the effectiveness of support interventions and gather feedback.
• Maintain accurate records of client interactions, referrals, and outcomes.
• Collaborate with internal teams to ensure holistic service delivery.
• Contribute to program development based on community insights.
