Responsibilities

• Develop and implement personalized care plans in collaboration with clients.

• Facilitate access to community resources including healthcare, education and employment services.
• Advocate for clients' needs within systems and institutions.
• Maintain accurate, confidential client records and progress reports.
• Organize and facilitate community outreach programs and workshops.
• Collaborate with social workers, healthcare professionals and community leaders.
• Respond to crisis situations with promptness and professionalism.
• Monitor client progress and adjust support plans as necessary.
