Qualifications

• 3+ years in a social care or community support role.
• Relevant qualification in Social Work, Psychology, or related field.
• Strong communication and active listening skills.
• Valid driver’s license preferred.

Responsibilities

• Develop and implement individualized support plans for community members.
• Facilitate access to healthcare, education and employment opportunities.
• Organize and lead community engagement activities and support groups.
• Conduct regular assessments to monitor progress and adjust strategies.
• Maintain confidential records of client interactions and progress.
• Collaborate with external agencies and local authorities to ensure comprehensive care.
• Provide emotional support and advocacy for individuals and families.
• Contribute to development and evaluation of community programs.
