Job summary

Placements24 is seeking a motivated Social Support Facilitator to join their Randburg team. This hybrid role combines on-site client interactions in Randburg with remote administrative duties to support individuals and families facing social challenges.

You will connect clients with community services, guide action planning, and facilitate group sessions, while maintaining confidential records and collaborating with other service providers to empower the Gauteng community.

Qualifications

• Relevant qualification in Social Work, Community Development, or Psychology.
• Experience in social support, case management, or community outreach.
• Strong communication, empathy, and listening skills.

Responsibilities

• Conduct needs assessments to identify social and practical challenges.
• Provide information, guidance, and support to clients.
• Facilitate referrals to housing, employment, healthcare, and counseling.
• Assist clients in developing action plans to meet goals.
• Organize and facilitate group support sessions and community activities.
• Offer practical support such as accompanying clients to appointments.
• Maintain confidential client records and progress notes.
• Collaborate with other social service providers for coordinated support.
• Advocate for clients' needs with service providers.
• Stay informed about available community resources and programs.
