Job summary

Qualifications

• Matriculation certificate or equivalent; a qualification in social work, community development, or a related field is preferred.
• Proven experience (minimum 2 years) in a community support, social work, or client service role.
• Valid driver's license and reliable transportation for community travel.

Responsibilities

• Engage with community members to understand their needs and challenges.
• Provide information and guidance on accessing local social services, healthcare, and other resources.
• Assist clients in completing applications and forms for relevant services.
• Facilitate referrals to appropriate agencies and service providers.
• Build and maintain positive working relationships with community members and service providers.
• Conduct follow-up with clients to ensure they have received the necessary support.
• Organise and participate in community awareness events and information sessions.
• Maintain accurate and confidential client records and service logs.
• Report on community needs, service gaps, and engagement activities.
• Act as an advocate for clients within the service delivery system when appropriate.
